A Case of Delayed Propagation of Peristalsis in the Upper Esophagus

A 27 year-old female presented with a history of intermittent dysphagia for solids and liquids. Dysphagia developed 10 years previously and was aggravated two months prior to presentation. Esophageal manometry showed markedly delayed propagation of peristalsis over the upper esophagus, with normal propagation over the mid- and lower esophagus. Following more than half of wet swallows, the peristaltic propagation velocity was 0.2~0.5 cm/sec along the upper esophagus. The reason for this phenomenon was not clear. It is very difficult to say whether it was due to slow propagation or delayed onset of primary peristalsis or non-transmission of the primary peristalsis with appearance of a secondary peristalsis.
